The Charlotte Family Health Center provides primary care for patients of all ages in a small, community-based setting. We provide health care that is driven by evidence-based guidelines, and we manage that care across multiple settings.

Office hours and urgent care

We schedule appointments Monday through Friday, 9 am – 4 pm. On weekdays our phone lines are active 9 am – noon and 1:30 – 4:30 pm. For your convenience we also schedule appointments on occasional Saturdays, 9 am – noon.

Most acute problems are best addressed in person. We are committed to providing timely care for urgent problems, and we reserve some of our routine appointment times for this purpose. When we can, we will schedule same-day appointments to treat acute illnesses, evaluate and treat minor injuries, or stitch lacerations, thereby saving you a trip to the emergency department.

We strive to return urgent phone calls from patients promptly, 24 hours a day, 7 days a week. Outside regular office hours, a practitioner is on call to assist in urgent matters; please call our answering service at 802-350-6895. If you are having a medical emergency, please dial 911 immediately.

Choosing a primary care physician

Our patients choose a primary care physician who will coordinate their health care, including test results, referrals, records, and other information. If you do not have a preference, we will be happy to help you with your choice.

Medical home

We will coordinate your care with other physicians who provide specialized care, hospitalists should you be admitted, and other providers of health care services such as physical therapists, dieticians, visiting nurses, and nursing home personnel. Our practice will function most effectively as a medical home if patients and families provide us with a complete medical history and information about care obtained outside the practice.


We provide comprehensive primary care health services for all ages including:
• Care of acute and chronic illnesses
• Complete physical exams for adults, teens, and children
• Well-child pediatric exams
• Camp and school physicals
• Flu vaccines and other immunizations
• Routine treatment for illness and infection
• Evaluation and treatment of injuries, including stitching of lacerations
• Minor skin surgery, including biopsies and freezing
• Lab tests, including PT/INR, lipid panels, diabetes monitoring, urinalysis, and strep tests
• Women’s health care
• Adolescent health care
• Self-management support

Prescription refills

Your condition and your response to medication can change over time. If you have a chronic disease that requires long-term medication, we will give you enough prescription refills to last until the time of your next recommended visit. Please call the office for an appointment well before your prescription runs out. In order to provide excellent care, we require all patients on long-term medication to be seen at least once each year.

In case of an unusual refill requirement between visits, we will prescribe sufficient medication to tide you over until your next visit. Please call the office and allow 3 days for your routine prescriptions to be processed, and up to 10 days for those requiring prior authorization.

Please plan ahead to avoid gaps in your prescription medication coverage. Due to changes in insurance coverage, some of your routine medications may now require prior authorization, have a limited number of refills annually, or may no longer be covered.

Often lower-priced generic medications will work well. If cost is an issue, please ask us about alternatives to the more expensive drugs.

Insurance and payment

We accept and bill Medicare and Medicaid and most private insurances. Co-payments are due at time of your visit and can be made with cash, check, Visa, or MasterCard. We offer a prompt-payment discount to patients who pay in full at the time of visit.
